Accessing MRI Services

Step 1

To schedule an examination, we require a requisition. Please download this REQUISITION FORM  and take it to your health care provider to fill out.

The following health care providers can fill out your requisition form:

  • Your family doctor
  • A medical specialist
  • A chiropractor

We also accept requisitions from other medical facilities.

Step 2

Once your requisition for an MRI is received, we will call you to schedule your appointment. In many cases, your MRI can be done within a matter of days and results back within 24 hours.

Step 3

Fill out the Client History and MRI Screening Questionnaire  and bring this with you to your appointment.

Step 4

Read the Patient Information for Magnetic Resonance Imaging (MRI) article with information on precautions, preparation and what to expect.

NOTE: A consult with one of our Medical Specialists can be arranged to correlate your symptoms and MRI images. Please call the Program Coordinator at 250.828.6740 to discuss this option.

If you are unable to attend your appointment, please call the Welcome Back Pain Management & Upright MRI Centre at 250-828-6740 or 1-866-759-2674 (Toll Free in North America). Please note: a fee may apply if you do not cancel your appointment within 24 hours of your scheduled time.
